Participate in the Alliance

First Coast Health Alliance is an opportunity for physicians to retain their independence while also participating in innovative partnerships and achieving the following goals:

• Adoption of evidence-based standardized care protocols
• Reduction of health care costs for patients, employers and health plans
• Enhanced coordination of care between all care providers across the continuum of care
• Assuring delivery of the right care at the right time in the right setting
• Improved reimbursement through demonstration of quality and efficiency
• Improved clinical outcomes for all patients

If you are interested in participating First Coast Health Alliance, please review the Operating, Subscription and Participation Agreements that are posted below. These documents have been reviewed by legal counsel obtained by the physician members of the PHO board and members are satisfied that the contents of these documents sufficiently represent physician interests.
